Home Based Primary Care Program

If you or someone you know is:

* An enrolled veteran and has one or more of the following medical conditions:
  • COPD
  • CHF
  • Diabetes
  • Neurologic Disorder (ALS, MS, Parkinsons)
* Are home-bound or have difficulty getting to your primary care appointments because of your medical condition.
* Live 30-40 minutes drive from Lebanon VA

You may be eligible to receive primary care visits in your home. For more info, ask your provider or call 717-272-6621 # 4406.

You do NOT have to be service connected to receive this care!

Did you know that there are 1,192,000 cell phone subscribers in Iraq and phone use has gone up 158%?

Did you know that Iraq has an independent media that consists of 75 radio stations, 180 newspapers 10 television stations?

Did you know that the Baghdad Stock Exchange opened in June of 2004?

Did you know that 2 candidates in the Iraqi presidential election had a televised debate recently?

Instead of reflecting our love for our country, we get photo flag burning incidents at Abu Ghraib and people throwing snowballs at the presidential motorcades.
